This is a full-time position with weekday hours. Occasional evening duties or activities are required.

Service Territory could include all counties serviced by Stillwater Hospice, focusing on the Auburn, Kendallville, Warsaw, and Columbia City areas.

POSITION TITLE: HOSPICE CARE CONSULTANT


FLSA STATUS: Exempt


REPORTS TO: Business Development Team Lead


POSITION OBJECTIVE:

  • The Hospice Care Consultant will provide end-of-life care education to and obtain referrals from healthcare professionals in hospitals, home health agencies, nursing facilities, long-term care facilities, and physicians.


QUALIFICATIONS:

  • Bachelor’s degree preferred.
  • Proficient in Microsoft Office Suite.
  • Previous experience in hospice, home health, healthcare sales, or marketing.


ESSENTIAL FUNCTIONS:

  • Exhibits excellent verbal and written communication skills.
  • Must be an autonomous self-starter with a high level of accountability and responsibility.
  • Must be flexible, highly organized, and able to prioritize their work.
  • Performs intentional marketing calls while networking with physicians, skilled nursing and assisted living facilities, and hospital discharge planners, building relationships and promoting awareness of our services and referrals to our programs.
  • Seeks out new referral source contacts while maintaining relationships that meet agency values.
  • Meet the performance goals associated with the accounts and territories assigned.
  • Manages daily and weekly schedule of marketing activities per requirements. Analyze territory information to optimize routing and achieve results. Monitor local market conditions for changes that impact business.
  • Utilizes effective marketing skills and product knowledge to influence targeted healthcare professionals. Executes brand strategies to ensure a consistent company sales and marketing message.
  • Utilize marketing tools, resources, and supporting analysis to plan activity. Develop and execute plans to maximize marketing resources.
  • Perform Company business in accordance with all regulations, company policy, and procedures. Demonstrate high ethical and professional standards at all times.
  • Collaborates with the Admission Department regularly on the status of all assessments and pending referrals and reports pertinent data regarding admissions and pending referrals to their referral sources.
  • Proficient, timely, and up-to-date in all aspects of documentation, including computer data entry into patient electronic records and the Customer Relations Management (CRM) program.
  • Builds trust, values others, communicates effectively, fosters innovation, focuses on the customer, collaborates with others, solves problems creatively, and demonstrates high integrity. Displays compassion, works as a team player, has an optimistic outlook, and is adaptable, accountable, and customer-driven.
  • Adheres to all Agency policies and procedures.
  • Participates in quality improvement activities and/or other agency committees or task forces when requested.
  • Must be able to safely perform the job's essential functions without posing a direct threat to the health and/or safety of self or others in the workplace.
  • Must have an Indiana motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
  • Adheres to Stillwater Hospice privacy practices as described in the Notice of Privacy Practices.
  • Any other duties as directed by the supervisor.


GENERAL FUNCTIONS:

  • Maintains confidentiality of personal matters related to patients, co-workers, and Agency business.
  • Work in a cooperative relationship with other Agency personnel.
  • Seek appropriate supervision.
  • Seek educational opportunities and attend in-service programs to continue developing personal growth.


ESSENTIAL PHYSICAL/MENTAL FUNCTIONS:

  • Must have visual, hearing, and mental ability to initiate and comprehend written and verbal communication.
  • Must be able to engage in clear, concise verbal/non-verbal communication.
  • Must be able to perform tasks involving physical activity such as lifting 50 pounds maximum with frequent lifting and/or carrying of objects weighing up to 25 pounds; walking, standing, talking, hearing, pushing, fingering, reaching, seeing, stooping, smelling, depth perception, and color vision.
  • Must be able to climb a minimum of three (3) flights of stairs.
  • Must be able to endure long periods of sitting while driving.
  • Must have the ability to remain calm in occasional emergency situations.


ESSENTIAL HAZARDOUS PHYSICAL EXPOSURES:

  • Must be willing to travel in inclement weather.
  • The potential threat of physical violence to self.
  • Potential exposure to environmental pollutants.
  • Potential exposure to infectious diseases.
  • Potential exposure to variable temperatures in the working environment.


CHEMICAL OR HAZARDOUS PHYSICAL EXPOSURES:

  • Potential exposure to substances within the office environment, including copy machine toner, VDT emissions, cleaning solvents, and medical supply chemicals.


ESSENTIAL PERSONAL PROTECTIVE EQUIPMENT REQUIRED:

  • According to state laws, seat belts must be worn when traveling during working hours. Universal blood and body fluid precautions will be consistently used for all patients.


OTHER SPECIAL CONSIDERATIONS:

  • Must be willing to accept assignments and other duties as deemed necessary without regard to location, social, economic environment, disease, or ethnic origins.
  • When assigned, you must be willing to complete enrollment visits on Holidays, weekends, and evenings.


The published compensation range for this position reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The base pay may be higher or lower based on demonstrated job-related experience.
