Compensation is based on total years of experience in previous roles.

This role is in Fort Wayne, Indiana, at the Stillwater Campus on Homestead Road.

You will be working 40 hours each week. Monday through Wednesday 3p-11p plus every Saturday and Sunday 8a-430p.

POSITION TITLE: ON-CALL COORDINATOR

FLSA STATUS: EXEMPT

REPORTS TO: CLINICAL TEAM LEADER- SUPPORT PROGRAMS

POSITION OBJECTIVE:

  • Coordinates and supervises after-hours home health and hospice services to individuals and families in the home care setting within the Stillwater Hospice service area.

QUALIFICATIONS:

  • Must have a current LPN or RN license to practice in Indiana.
  • Graduate of an accredited school of nursing or LPN program.
  • Must have at least two (2) years of recent clinical experience, preferably with experience in home care.
  • Must be comfortable working with home health and hospice patients with an emphasis on palliative care.
  • Currently certified in CPR, must obtain within six months of hire if not certified.
  • Currently certified in TB, must obtain within six months of hire if not certified.
  • Must have interpersonal skills to supervise staff and interact with patients and family members effectively; must exhibit good public relations skills for interaction with physicians and other healthcare community members.
  • Must be able to plan and implement changes and flex between roles as necessary.
  • Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
  • Must drive independently to all patients'/family clients’ homes.
  • Must be able to safely perform the job's essential functions without posing a direct threat to the health and safety of self or others.

ESSENTIAL FUNCTIONS:

  • Interview, hire, orient, and supervise on-call home care staff
  • Supervises assigned Skilled Nurses with adherence to requirements mandated by appropriate third-party payers, regulations, and agency policies.
  • Assures continuity of care through interaction with staff.
  • Makes supervisory visits with home care staff to ensure requirements are met.
  • Works with Triage and phone nurses to ensure there are appropriate staff assignments and scheduling for patients in designated service areas.
  • Work with support service leaders to oversee the assigned clinical team.
  • Assures that documentation in the patient record is accurate, objective, comprehensive, and verified in the required time.
  • Works closely with third-party Triage company as point nurse
  • Manages the monthly on-call schedule
  • Performs visits, takes referrals, and admits home care patients, if necessary, to supplement staffing needs.
  • Participates in continuing education to constantly improve nursing and supervisory skills.
  • Attend various committee meetings held throughout the agency.
  • Participate in quality improvement projects as assigned by the supervisor.
  • Participates in the concern process by completing investigations and working with the Clinical Team Leader:
  • Support programs in planning for improvements to the home care concerns assigned to their team.
  • Assumes other responsibilities as assigned by supervisor.
  • Adheres to all Stillwater Hospice policies.
  • Adheres to Stillwater Hospice privacy practices described in the Notice of Privacy Practices.
  • Proficient in software used routinely, including but not limited to the electronic medical record, Outlook, Excel, OneDrive, etc.

CRITICAL SKILL SETS & ATTITUDES:

  • Proficient and timely in all aspects of clinical documentation, including computer data entry.
  • Proficient in blood draws, routine patient treatments (dressing changes, catheters, etc.), pain and symptom management, patient and family care, including pediatric and HIV patients, and Mantoux administration.
  • Proficient in all aspects of IV care, including programming of IV pumps, tubing changes, site care, and subcutaneous access.
  • Display compassion, be a team player, have an optimistic outlook, and be adaptive, accountable, and customer-driven.

OTHER SPECIAL CONSIDERATIONS:

  • Must be willing to accept assignments without regard to location, socioeconomic environment, disease, or ethnic origin.
  • Must be willing to make scheduled visits on weekends or evenings when assigned.

The published compensation range for this position reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The base pay may be higher or lower based on demonstrated job-related experience.
