About the Position :
Under direction, performs work of moderate difficulty by providing skilled casework services to selected caseloads or clients with special problems such as health disability or those at risk of nursing home placement; provides specialized casework services aimed at securing the client's overall well- being and maximum degree of independent functioning. Serves large geographic areas which may Traditional / Enhanced EDWP Case Management XVIII-204 include one large county and / or many small counties which may involve extensive travel, and performs related work as required.
MAJOR JOB DUTIES AND RESPONSIBILITIES :
MINIMUM JOB REQUIREMENTS
Education & Experience : Minimum Education and Experience : Bachelor's degree in social work, sociology, psychology, or a related field, with two years’ experience in the human service or health related field, or Master’s degree in social work, sociology, psychology, or a related field with at least two years’ experience.
Certification : CPR Certification required.
Knowledge / Training : Must have the ability to effectively coordinate and communicate with members, EDWP service providers, general public and other team members. Be able to establish and sustain interpersonal relationships by having the knowledge of human behavior or gerontology. Must have the knowledge and skill in social and health service intervention techniques and methodology. CCSP / SOURCE knowledge required.
Computer Skills : Must be proficient with general use and functions of computer technology such as Basic Microsoft Applications, starting programs, using files and windows, navigation buttons and tool bars.
Transportation : Must have reliable transportation.
