The programs within Community Care allow senior adults and people with functional and / or cognitive impairments to stay in their homes and communities for as long as possible. CCSP and SOURCE are Medicaid Waiver programs managed by the Georgia Department of Community Health. As a provider of case management services, Visiting Nurse, supports several home and community-based services (HCBS). While there are three distinct programs, each one is designed to help people with disabilities, or the elderly, remain in their community instead of living in an institution. Visiting Nurse Health System provides case management oversight for over 4,000 clients in the Greater Atlanta, Greater Rome and Greater Macon areas at any given time allowing them access to community-based care while retaining independence.
About the Position
The Executive Director (ED) of Community Care will be a high energy, team oriented, inspiring leader who builds strong relationships within a diverse team. Leading the overall strategic direction and operational oversight of the Community Care division, the ED will direct Community Care operations in accordance with the policies, procedures, mission, and philosophy of VNHS. The Executive Director is responsible for planning the efficient delivery of consolidated case management and assuring that consolidated case management objectives for the programs are met. The ED will possess a strong knowledge of Medicaid and LTSS standards and healthcare regulations. Responsibilities include managing all aspects of two multi location Medicaid Waiver programs, as well as an Older Americans Act funded program. Assists in the development of the budget and monitoring to ensure the Program functions within budgeted guidelines. This leader will collaborate with senior leadership and report directly to the President and CEO.
Responsibilities :
The Executive Director will oversee daily clinical operations, ensuring compliance with healthcare regulations and the highest of standards.
- Demonstrates proven decision-making, organization, and management skills
- Ensures that all medical management practices align with industry best practices, Medicaid standards and the highest standards of client care and safety
- Analyzes data and implements evidence-based practices while monitoring continuous quality improvement strategies
- Demonstrated knowledge of program policy and procedures
- Responsible for assisting in budget development and delivery of financial outcomes
- Manage and supervises clinical staff, oversees productivity goals, provides guidance, support, performance evaluations to plan and monitor professional development and adherence to protocols
- Engage in appropriate written and verbal communication with your team, senior leadership, clients, families and insurance companies
- Strong problem-solving skills and the ability to make informed decisions under pressure
- Performs quality assurance audits on all clinical records while driving important workflows and quality outcome
