Visiting Nurse Health System (VNHS) is hiring for a compassionate, dedicated and skilled RN Case Manager in Hospice Division serving patients at two senior living facilities in the Chamblee / Buckhead area.

About the Position :
A Hospice registered nurse (RN) provides compassionate care to patients who are terminally ill. The main goal is to help patients feel comfortable and dignified during the final stages of their life. Hospice Nurses treat patients with complex care needs, at the end of life, and depend on their Nurse Case Manager to provide symptom relief, formulate care plans, maintain communication with other disciples, schedule appointments, and provide education on end-of-life care. Hospice RN’s, also, provide emotional support to the patients and their families. As a RN Case Manager, you will coordinate your patients plan-of- care with an interdisciplinary team of healthcare professionals including doctors, nurses, social workers, chaplains, and volunteers to provide comprehensive care. This team will address the patients physical, emotional, spiritual, and social needs through coordinated efforts and regular communication across disciples.
Must have qualifications :
- A valid Georgia RN License
- 3 years of RN experience in a Hospice, Medical-surgical, ICU, ER or Oncology environment
- Strong clinical assessment skills
- Wound Care skills
- Reliable transportation and valid auto Insurance
