In this position, you will :
Work as a Registered Nurse (RN) for our dynamic home healthcare team, responsible for providing direct patient care under the supervision of the Clinical Manager. This position is responsible for coordinating the care delivered to a caseload of home bound patients and ensures care is delivered within quality and utilization guidelines. You will assess, plan, implement, monitor and evaluate actions taken as part of a comprehensive plan of care to ensure outcomes of care meet or exceed standards.
As the RN, you'll provide skilled nursing care in the home setting for an assigned case load of patients. You'll work one-on-one with your patients and their families providing patient education, and clinical nursing observations and documentation for home health patients.
What you need to join our team.
- A valid Georgia RN License
- 2 years of RN experience in a med / surg, icu, or ER setting. Home Health experience preferred
- Strong clinical assessment skills
- Wound Care Experience
- A love for patient care and education
- Reliable transportation and valid auto Insurance
