What are the responsibilities and job description for the Transition Specialist RN/LPN - Fulton County position at Visiting Nurse Health System Inc?
POSITION SUMMARY
Begins and/or completes the admission process within the acute care facility. Provides a written or electronic assessment of the patients needs for in-home services and/ or hospice. Coordinates initial home health care and/ or hospice delivery. Serves as a liaison for Visiting Nurse Health System to facility staff, physicians, patients, families and other organizations. Collaborates regularly with the Account Executive marketing initiatives within territory.
Achieves referral and admission retention and growth targets from referral partners in a defined territory. Referral partners include hospitals, nursing homes, long term care/assisted living facilities, physician offices, community agencies and/or rotation partners.
Collaborates with Account Executives on territory and account plans.
MINIMUM JOB REQUIREMENTS
Education & Qualifications:
Education: Graduate of an accredited Diploma, Associate Degree or Baccalaureate School of Nursing preferred or Licensed Practical Nurse, Masters or Bachelors of Social Work degree or Licensed Clinical Social Work.
Licensure/Certifications: Current licensure by the Georgia State Board of Nursing strongly preferred.. Must have current and valid driver s license.
Previous Experience: 2-3 years of experience in community health nursing and/or home health care and/ or hospice care and/or case management experience preferred for nursing candidates. 2-3 years of experience with hospice or hospital case management and/or discharge planning strongly preferred for all candidates.
Knowledge: Demonstrates knowledge of generally accepted nursing standards and regulatory requirements and techniques of home health nursing and/ or hospice care. Knowledge of all company services and eligibility requirements for those services; third party coverage. Clerical ability to operate a variety of office machines and competent keyboarding skills. Strong multi-tasking abilities, strong communication skills and ability to work well with physicians, case managers, social workers, patients and co-workers are a must.
Skills: Excellent oral and written communication skills to take and write orders. Establish and maintain good working relationships with facility staff and physicians. Ability to identify and respond to VNHS business opportunities within the facility.
Computer Skills: Microsoft Word, Excel and Outlook proficient. Previous experience with McKesson: Horizon Homecare preferred
Transportation: Must have reliable transportation and valid automobile insurance.
Work hours & days: Candidates must have the ability and flexibility to work varied hours (not to start earlier than 8am and end no later than 8 pm) and to work weekends and holidays on a rotation basis.
