What are the responsibilities and job description for the Hospice RN Case Manager - Cobb/Cherokee position at Visiting Nurse Health System?
Responsible for the care management and coordination for hospice patients and families. Works collaboratively with Medical Director and interdisciplinary team; provides direct nursing care, emotional support and education to terminally ill patients and their families. Demonstrates clinical expertise in nursing assessment, nursing diagnosis, and advanced proficiency in technical skills necessary for developing, providing, and coordinating care. Working in Senior Living Facilities with Hospice Savannah.
Education & Qualifications:
Education & Experience: Graduate of an accredited Diploma, Associate Degree, or Baccalaureate School of Nursing. Preference is given to individuals with Baccalaureate Degree and national certification in Hospice and Palliative Care. Minimum of 3 years professional experience in critical care, medical-surgical nursing, Hospice, Oncology, or community health nursing. Home hospice experience preferred.
Licensure/Certifications: Must have current licensure by the Georgia State Board of Nursing and a current valid Driver's license.
Knowledge/Training: Knowledge of the physical, social, and emotional needs of the elderly disabled and terminally ill. Knowledge of Medicare, Medicaid and other funding source regulations as applied to hospice care services. Demonstrate knowledge and competence in advanced practice nursing skills, including the nursing process, and collaborating with interdisciplinary team. Skill in communications in order that information can be exchanged effectively with persons at various educational and attitudinal levels. Knowledge of supervision to effectively relate with volunteers and staff.
