Under the direction of the Clinical Manager, our Nurse Case Managers provide skilled nursing care to our patients as prescribed by the Physician and in accordance with the policies and procedures of the Visiting Nurse Home & Hospice. Our Nurse Case Managers provide care in a manner that promotes quality, continuity and safety for the patient and their family by making initial and ongoing assessments of the patient and family needs.

Nurse Case Manager Qualifications:

  • Comprehensive knowledge of current nursing practice.
  • Ability to make meaningful observations and to complete concise, accurate reports.
  • Thorough knowledge of and ability to interpret Agency program and policies to patient/family and public. Helps patient's family or significant other accept responsibility for providing care.
  • Ability to exercise independent professional judgment and to effectively oversee and manage patient care and function in the role of Case Manager.
  • Ability to relate objectively with co-workers, patient/family, other professionals in the Agency and in other agencies and the public.
  • Ability to recognize needs and concerns of people and to deal constructively with them.
  • Knowledge of and ability to use community resources.
  • Ability to study and evaluate own job performance and to plan and carry out program for continuing professional growth.
  • Ability to demonstrate competency in adult phlebotomy.
  • Maintains current RN licensure in Rhode Island and Massachusetts and obtains necessary CEU’s to maintain licensure.

Education and Experience:

1. Graduation from a school of nursing accredited by NLN:

  • From a baccalaureate or master’s degree program in nursing.
  • From a state approved diploma or associate degree program in nursing.

2. Nurses without a baccalaureate degree must have four years experience in nursing. Staff are encouraged to pursue their education toward a baccalaureate degree in nursing. (This can be waived if determined by CEO.)

3. Registered as a professional nurse in BOTH Rhode Island and Massachusetts. If not licensed in Massachusetts at time of hire, RN will have 1 year from hire date to obtain Massachusetts RN license.

4. Interest in keeping abreast of trends and developments in the nursing field.

5. Current certification in CPR.

Job Specifications:

  • Hourly / Non-Exempt / Part-Time, 32 Hours/Week
  • Every 4th weekend (1 per month) rotation required per department needs.
  • Every other minor holiday rotation required per department needs.

Job Type: Part-time

Pay: $36.00 - $44.00 per hour

Expected hours: 32 per week
