Our Agency is seeking an EXPERIENCED REGISTERED NURSE - HOSPICE CASE MANAGER for the Farmington Valley area to join our progressive interdisciplinary team of clinicians, chaplain, social work, home health aides and volunteers. The RN Case Manager will have a caseload of palliative and hospice patients and will serve as the clinical liaison for hospice at UConn Medical Center. The agency adheres to National Best Practice Guidelines for caseloads and visit schedules. We offer a flexible work environment and a competitive benefits package. Weekend and holiday rotation.
