What are the responsibilities and job description for the Spiritual Counselor position at Visiting Nurse & Hospice of Litchfield County?
Visiting Nurse & Hospice of Litchfield County is looking for a Spiritual Counselor to join our well-established and warm interdisciplinary hospice team that serves patients and families in Northwest Connecticut. The Spiritual Counselor in collaboration with the Interdisciplinary Treatment Team (IDT), is responsible for providing spiritual and pastoral care to the patients and families that are being served by Visiting Nurse & Hospice of Litchfield County (VNHLC).
The Spiritual Counselor supports and fosters the philosophy and mission of the organization, understands the culture of hospice, offers compassionate care to the patient and family at various stages of their illness.
Essential duties include:
- Coordinate, monitor and document ongoing patient/family pastoral and spiritual care
- Participates in Interdisciplinary Team (IDT) meetings
- Updates IDT care plan addressing spiritual issues
- Contacts/coordinates with communities' religious care providers to assist in caring for the patient, family and caregiver as needed.
