**Job Description**: The successful candidate will be responsible for developing and implementing treatment plans that focus on the physical, cognitive, and emotional needs of patients. This will involve assessing home environments to ensure safety and providing recommendations for any necessary modifications or equipment needs. Additionally, the Occupational Therapist will provide patient and caregiver education, accurate and timely documentation, and work independently as part of a team.
