Role Responsibilities
Conduct home safety assessments and recommend necessary modifications or adaptive equipment
Develop comprehensive treatment plans to meet the unique needs of patients
Collaborate with multidisciplinary teams to ensure consistent patient care and outcomes
Requirements
Licensed Physical Therapist (PT) in Massachusetts
Experience with electronic medical records (EMR) systems
Home healthcare experience preferred, but not required
