PURPOSE STATEMENT:
Plan, direct, define and coordinate individualized and group therapeutic support and services to patient’s with
mental health, behavioral health and substance abuse issues.
ESSENTIAL FUNCTIONS:
- Conduct social history assessment of the patient through observation and patient/family interview,
implementation of the treatment plan.
- Coordinate and plan programs and activities to meet patient’s social and emotional needs.
- Promote recovery, symptom reduction and the achievement of the highest level of social functioning.
- Provides crisis management and family sessions to assist the family in understanding the implications
- Assist in the development of effective treatment plans that apply therapeutic principles appropriately
- Participate in multidisciplinary treatment team meeting to discuss patient assessment, treatment plan
- Communicate or aid family members in understanding and supporting the patient.
- Manage a caseload and assume responsibility for patient chart documentation.
- Facilitate education, process groups, support groups and referrals as requested.
- Demonstrate a positive, empathetic and professional attitude towards customers always. When patient
priority.
- Perform other functions and tasks as assigned.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
- Bachelor's degree in Social Work from an accredited college or university, including new graduate required (Masters Degree preferred).
- One or more years’ experience working with patient population preferred.

- Masters or better in Social Work
