Vista Grande Villa, located in Jackson, MI is currently seeking a PRN MDS Coordinator who will be responsible for coordinating and overseeing the Minimum Data Set (MDS) assessment process in a nursing home setting. They ensure accurate and timely completion of MDS assessments, collaborate with interdisciplinary teams, and maintain compliance with state and federal regulations.
Hybrid schedule; partially remote.
Responsibilities:
1. Coordinate and complete MDS assessments for all residents in accordance with state and federal regulations.
2. Collaborate with the interdisciplinary team to gather necessary information for accurate assessment completion.
3. Ensure timely submission of MDS assessments to the appropriate agencies.
4. Review and analyze MDS data to identify trends, patterns, and areas for improvement.
5. Develop and implement care plans based on MDS assessments and resident needs.
6. Train and educate staff on MDS assessment processes and documentation requirements.
7. Stay updated on changes in MDS regulations and communicate updates to the team.
8. Participate in quality improvement initiatives to enhance resident care and outcomes.
9. Maintain accurate and organized MDS records and documentation.
10. Serve as a resource for staff, residents, and families regarding MDS assessments and related matters.
Qualifications:
1. Bachelor's degree in nursing or a related field (preferred).
2. Valid nursing license (RN) in the state of Michigan.
3. Minimum of 2 years of experience in a nursing home or long-term care setting.
4. In-depth knowledge of MDS assessment processes and regulations.
5. Strong analytical and problem-solving skills.
6. Excellent communication and interpersonal skills.
7. Ability to work collaboratively in a team environment.
8. Proficiency in using electronic health record systems and MDS software.
