Job Summary:
Vita Nova is looking for a energetic, passionate and driven individual to help older homeless youth obtain and maintain employment so that they can live healthy and productive lives.
Job Purpose/Duties:
This position is part of Palm Beach County's larger initiative for older youth called Securing Our Futures Initiative Program. The goal is to support youth in maintaining or increasing their self-sufficiency in areas such as transportation, childcare, housing, employment, income, and education. Ideal candidates will provide support to youth with career exploration, build job readiness skills, and improve access to post-secondary opportunities. Training will be provided by Vita Nova to ensure success for the youth and team member.
Case Management Essential Functions:
- Complete initial intake, needs assessments, and other program enrollment forms and act upon critical needs appropriately and immediately
- Meet with youth to develop a comprehensive client-driven case plan with both short-term and long-term goals identified
- Provide individual assessments to increase skills and income
- Provide Life Skills/Financial Literacy Training to achieve Self-Sufficiency
- Assist Youth with Enrollment in GED or Vocational Training Program
- Complete self-sufficiency matrix, employment assessments, and all contractual documentation (i.e. case notes) for county's and other databases
- Periodic review of economic mobility and tracking progress through financial literacy, life skills, and completion of self-sufficiency matrix
- Orient youth to program services, requirements and eligibility.
- Arrange, coordinate, and transport youth to various appointments
- Connect with community partners who may be helpful to youth or with programming
- Provide professional, timely communication with Community Partners
- Assist in coordinating and facilitating life skills trainings/events including nights/evenings and weekends when necessary
- Integrate positive youth development standards and trauma informed care practices when serving youth
- Promote adult supportive connections by facilitating activities for youth to engage with mentors
- Participate in community meetings aimed at providing education on the needs of disconnected and homeless youth.
- Maintain client files to include initial intake, basic needs assessment, individualized plans, termination reports, progress case notes and other documents related to youth service delivery.
- Meet established deadlines for submitting progress case notes, file management forms, program reports/data and personnel documents.
- Collect data necessary to meet program outcomes and statistical reports timely.
Qualifications:
Education
- Minimum of a Bachelor Degree and related experience in human services field
Knowledge, Skills and Abilities
- Knowledge of case management and basic documentation
- Knowledge of working with youth/young adults and families
- Ability to provide services with a Trauma Informed Care approach and Positive Youth Development standards.
- A valid driver's license with a good driving record and ability to join agency's insurance policy
- Availability to work non-traditional hours including nights and weekends
- Ability to manage multiple assignments
- An ability to lift and move items such as furnishings up to 50 pounds
Proficiency in the use of computer programs for:
- word processing
- databases
- spreadsheets
- internet
- graphic design programs

Working Conditions
- Case manager will work primarily in the community, requiring extensive daily travel, while partnering with youth to accomplish goals
- Case manager will work non- traditional hours; evening and weekends are needed
- Case manager will work in a fast paced, often crisis oriented, program aimed at meeting the needs and goals of disconnected/homeless youth.
Salary : $47,000 - $50,000
