Vita Nova is looking for a passionate, driven, and energetic Healthcare Specialist to help older homeless youth access available healthcare services so they can living healthy and productive lives.
JOB PURPOSE:
Vita Nova's Healthcare Specialist will partner with former foster, homeless and disconnected youth ages 18-24 to assess current healthcare needs, then develop and execute an individualized plan to address those needs. Youth served by the Spot drop-in center, the Vita Nova Village transitional living program, and Independent Living Services will be provided with educational opportunities to enhance their overall health and wellness through connections to medical, emotional and physical health resources.
KEY DUTIES INCLUDE BUT NOT LIMITED TO:
- Orient youth to program and healthcare services, requirements and eligibility, if applicable.
- Complete initial intake, needs assessment and other program enrollment forms and act upon critical needs appropriately and immediately.
- Educate youth on effective scheduling/rescheduling appointments, securing medical/mental health providers and locating specialized services when needed.
- Arrange, coordinate and transport youth to various appointments as a support during medical/emotional crisis.
- Assist in coordinating and facilitating life skills trainings/events including some on evenings and weekends.
- Integrate positive youth development standards and trauma informed care practices when serving youth.
- Promote adult supportive connections by facilitating activities for youth to engage with mentors.
- Participate in community meetings aimed at providing education on the needs of homeless youth and partnering to establish new resources for youth.
- Create new community partnerships with medical/dental providers willing to provide free (pro-bono) or reduced cost services.
- Assist with the maintenance and administration of the agency's food pantry to ensure youth have regular access to healthy and nutritious options.
- Maintain client files to include initial intake, basic needs assessment, individualized plans, vulnerability assessments, progress case notes, referrals and health care follow-up.
- Meet established deadlines for submitting progress case notes, program reports/data and personnel documents.
- Collect date necessary to meet program outcomes and statistical reports.
REQUIRED QUALIFICATIONS:
- Bachelor's Degree in Public Health or Human/Social Sciences
- 2 years' experience in delivering services to underserved populations
- Understanding of case management and basic documentation
- Knowledge of HIPAA requirements for safeguarding confidential information
- Awareness of Trauma-Informed Care Principles and Positive Youth Development standards
- Ability to work non-traditional hours including some nights and weekends to meet client or agency needs
