Job Purpose/Duties: Provide case management services to youth formerly in foster care, including those participating in the Postsecondary Education Services and Support (PESS) program, Aftercare Services and Support program, and/or Foster Youth to Independence (FYI) HUD program. Assess needs, create individualized plans, identify resources and provide life skills training to youth. Connect youth to education, employment and supportive adult connections. The program will focus on four core outcome areas: stablishing stable housing, education/employment attainment, creating permanent connections, and an increase in social and emotional well-being.
Essential Functions:
Case Management
- Orient youth formerly in foster care to program services, requirements and eligibility.
- Provide an innovative approach to problem solving that explores alternative options including accessing community resources.
- Complete initial intake, needs assessments, and other program enrollment forms and act upon critical needs appropriately and immediately.
- Process program applications for financial assistance timely and per state/federal guidelines.
- Provide aftercare supports, follow up and document in accordance to administrative code.
- Develop a comprehensive client-driven plan with both short-term and long-term goals identified by the youth.
- Arrange, coordinate, and transport youth to various appointments as needed.
- Connect with community partners who may be helpful to youth or with programming.
- Assist in coordinating and facilitating life skills trainings/events including some on nights/evenings when necessary.
- Integrate positive youth development standards and trauma informed care practices when serving youth.
- Promote adult supportive connections by facilitating activities for youth to engage with mentors.
- Adhere to documentation standards including documenting and tracking service delivery, referrals, all collaterals, and remote or face to face contact with each youth. All documentation must be inputted into the FSFN system within 48-hours of activity.
- Meet established deadlines for submitting case notes, program initial and renewal applications, file management forms, progress reports/data.
- Facilitate staffing between the lead agency and the young adult interested in opting-in to Extended Foster Care or other eligible programs.
- Participate in case transfer staffings, assessing needs and advocating for supportive services.
- Facilitate regular consultations with youth, lead agency and supportive adults to review progress and provide resources to address issues or concerns.
- Develop and implement projects aimed at producing training, tools, resources, and opportunities for youth to meet educational outcomes.
- Collect data necessary to meet program outcomes and statistical reports.
Qualifications
Education
- Minimum of a bachelor's degree in a human service-related field
- Two years' experience working with children in the dependency system.
Knowledge, Skills and Abilities
- Knowledge of case management and basic documentation
- Knowledge of working with youth/young adults and families
- Valid driver's license and good driving record
- Ability to manage multiple assignments
- Availability to work non-traditional hours including nights and weekends
- Ability to pass Level 2 background check and drug screen
Salary : $46,000 - $50,000
