APN / PA to Perform House Calls in Camden County
Primary Responsibilities :
Initiate and complete detailed patient assessments & physical exams
Conduct wellness exams and sick visits
Evaluate and manage common and acute chronic diseases
Order, interpret, and evaluate diagnostic and lab results
Refer and collaborate with other providers
Prescribe pharmacological and non-pharmacological therapies
Requirements :
Computer literate & EMR proficiency
Unrestricted APN license in the State of NJ with at least 1 year of clinical experience
Current DEA / CDS
Willingness and ability to work independently
Personable, passionate, and energetic
Ability to travel within Camden County
Previous experience with Medicare-eligible patients is a plus
Bilingual a plus
