Our busy home health agency is seeking an experienced home health aide to assist our case managers in implementing patient-specific care plans. You’ll give our patients companionship and help with basic medical care, light housekeeping duties, personal care, and hygiene tasks so that they are empowered to live happily at home. You must have 1-3 years of experience as an HHA or caregiver and possess excellent communication skills. Qualifications:
$16 - $18 hourly
Responsibilities:- Mitigate issues with medications by helping to open bottles, organizing self-administered dosing, and arranging for refills, and help the patient with meal preparation and planning
- Execute basic medical tasks such as replacing bandages and dressings, taking vital signs, measuring glucose and ketone levels, and ensuring the proper use of medical devices
- Drive patients as needed, for grocery shopping, running errands, or doctors’ visits, when necessary
- Sustain quality companionship and provide psychological and emotional support for patients and their family members
- Implement care plans as directed by the case manager, and keep updated and accurate care records
- Administer Medications: Dispense medications as directed by the Registered Nurse (RN), maintaining an annual error rate of no more than one mistake.
- Resident Response: Attend to resident call lights within 10 minutes to ensure responsive and attentive care.
- Light Housekeeping: Perform regular light housekeeping in resident rooms at least once weekly to promote a clean and comfortable living space.
- Service Charting: Accurately document all services provided without missing tasks, ensuring compliance with company standards.
- Assistance with ADLs: Support residents in Activities of Daily Living (ADLs) according to each individual’s Client Service Plan, with a zero-tolerance policy for missed tasks.
Qualifications:
- Capable of lifting up to 100 lbs, you may also be required to stoop, bend, reach, and transfer patients up to 250 lbs as needed
- Pass a full employment background check, drug test, and physical, and possess current CPR certification and first aid training
- Completion of home health aide training programs from community colleges are acceptable - Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) is a plus
- Some college preferred, but a high school diploma or GED is required
- Excellent communication and organizational skills are valuable, as is a compassionate, empathetic, and kind personality
