VITAE - Nurse Practitioner Program
The Position
As we continue to grow, we are looking for “SNF-ists,” psychiatric n urse practitioners that will provide care for the geriatric population in a Skilled Nursing and / or Assisted Living setting and bring quality mental health services to those communities.
What You Will Do
As the psychiatric nurse practitioner, you will act as a Skilled Nursing Facility’s leading provider of psychiatric services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents.
You will work with the entire facility’s patient population - short and long-term residents - overseeing their care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.
As a psychiatric nurse practitioner, you will provide types of care, including but not limited to :
- Evaluation of new-admission visits
- Coordination of discharge as needed
- Monthly stability visits for long term residents
- Evaluations for patients exhibiting changes in condition
