Role Description
This is a full-time on-site role for a Home Infusion Pharmacist located in Crown Point, IN. The Home Infusion Pharmacist will be responsible for dispensing medications, collaborating with healthcare providers, monitoring patient responses to therapy, and ensuring safe and effective infusion therapy for patients.
Qualifications
Sterile product
- compounding and dispensing skills
- Patient assessment and monitoring skills
- Strong communication and collaboration skills
- Knowledge of and experience with USP <797> guidelines
- Attention to detail and accuracy
- Active pharmacy license in Indiana
- Experience in home infusion or long-term care pharmacy is a plus
- Doctor of Pharmacy (PharmD) degree
