How You Will Make a Difference
As we expand our services in new and existing markets, we're looking for a Field Supervisor who shares our values of being Trustworthy, Capable, Compassionate, Proactive, and most importantly, Called to serve patients in their home. The Field Supervisor (LVN) plans, organizes and directs private duty nursing services in concert with our Values: trustworthy, capable, compassionate, proactive and called. The Field Supervisor (LVN) provides nursing care, with emphasis on health education to meet the needs of patients and families within their homes and communities. As the Field Supervisor, you will:
- Responsible for leading the overall field staff team efforts with positive attitude and in a professional manner, as well as creating and utilizing reports to monitor productivity and compliance of team members
- Communicate with manager regarding, but not limited to Reports, Team needs, Team Concerns
- Responsible for accurate and timely preparation and processing of PDN department required documents; coordinates care of clients and utilizes task planners appropriately in Clear Care
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient
- Prepares clinical notes in accordance with professional licensing body and internal policy using the company’s electronic medical record and submits documentation timely
- Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required
- Participates in on-call duties as defined by the on-call policy and based on needs of the company

Experience to Deliver on Our Mission
- Graduate of an accredited licensed vocational school of nursing and have a current licensure in state of practice and CPR certification
- Minimum one years home health, hospice, or related experience required
