Case manager for Behavioral Health
Vital Health is a full-service behavioral health and primary care center for adults and children in Northwest Ohio. Vital Health is looking for a caseworker to provide support in our programs for school-age children. The right candidate will have experience working with patients from culturally diverse backgrounds. An NPI and Medicaid provider number is required to create appropriate documentation.
This is a full time Monday through Friday position.
Essential Duties and Responsibilities
- Work with patients who are currently involved in IOP/Day treatment programs
- Use a strengths-based and trauma-informed approach to support behavior modification and life-skill development.
- Facilitate goal setting and treatment planning.
- Work with social workers and medical team.
- Strong presence in the community.
- Create treatment plans for patient.
- Facilitate mental health and substance abuse groups.
- Address barriers that impacts the ability of clients to meet goals.
- Assist with transportation.
- Work collaboratively with clients, their families, internal and external team members
- Willing to work in the community and with community partners.
- Provide linkage with community resources and referrals.
- Document all activities in a timely and accurate manner.
- Maintain compliance with agency policies and procedures, the Quality Assurance Plan, federal and state regulations, and healthcare industry standards
- Participate in assigned meetings, staff development, and training sessions
Requirements
· 3 or more year(s) case management experience.
· 2 or more year(s) substance abuse and mental health.
· Familiarity with QMHS documentation.
· Active CDCA.
· Previous Ohio Medicaid provider.
· Working knowledge of community resources
· Strong interpersonal and engagement skills
· Strong organizational and time-management skills
· Excellent written and verbal communication skills
· Strong computer skills required; experience with documentation in Electronic Health Record
· Ability to support the agency’s mission and philosophy
· Sensitivity to cultural diversity and workplace harmony
· Ability to deliver excellent customer service
Job Type: Full-time
Pay: $16.00 - $22.00 per hour
