We are seeking a compassionate and detail-oriented **Patient Care Coordinator** to join our home care team. This role serves as a vital link between clients, caregivers, and healthcare professionals, ensuring that each client receives personalized, high-quality care. **Key Responsibilities:**
**Client Coordination & Support**
- Serve as the primary point of contact for clients and their families.
- Develop and maintain customized care plans based on client needs and preferences.
- Conduct initial client assessments and ongoing evaluations to ensure quality care.
**Scheduling & Caregiver Management**
- Coordinate caregiver assignments based on client requirements and caregiver availability.
- Ensure proper coverage for all scheduled shifts, addressing any changes promptly.
- Support caregivers by providing guidance and ensuring compliance with care plans.
**Communication & Documentation**
- Maintain accurate client records, care plans, and progress reports.
- Communicate effectively with healthcare providers, caregivers, and families.
- Address client concerns and resolve any issues promptly.
**Quality Assurance & Compliance**
- Monitor the quality of care and ensure compliance with company policies and state regulations.
- Conduct regular follow-ups to assess client satisfaction and adjust care plans as needed.
- Stay up to date with best practices in home care services.
**Qualifications & Skills:**
- Previous experience in home care, healthcare, or a related field preferred.
- Strong organizational, multitasking, and problem-solving skills.
- Excellent verbal and written communication abilities.
- Proficiency in scheduling software and electronic medical records (EMR) systems.
- Ability to work independently and collaboratively in a fast-paced environment.
- Compassionate and client-focused approach to care.
**Education & Experience:**
- High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare administration, social work, or a related field is a plus.
- Experience in a patient care coordinator or administrative role in home care or healthcare settings preferred.

Expected hours: 5 – 25 per week
Schedule:
- Day shift
- Evening shift
- Monday to Friday
- Rotating weekends
- Weekends as needed
