POSITION SUMMARY
Through the collaboration and supervision of a physician, the licensed Advanced Practice Nurse will provide professional medical services to patients in accordance with healthcare, nursing, medical and facilities regulatory standards including, but not limited to: perform physical assessments; develop, implement, and evaluate treatment plans; monitor aspects of patient care; collaborate with other healthcare disciplines; complete documentation that is accurate and submit in a timely manner; utilize effective and therapeutic communication between patients and their families, nursing staff, administrative personnel and other healthcare disciplines; conduct self in a professional and ethical manner; pursue opportunities that will enhance personal and professional growth of self in the specialty of Wound Care
OPERATIONAL DUTIES AND RESPONSIBILITIES:
- Establish diagnosis and formulate plan of care, as well as assess patient’s response to therapy
- Utilize evidence based research and advanced clinical skills to enhance patient outcomes
- Disease monitoring of pertinent chronic conditions and medications that may impact wound status or healing such as peripheral vascular disease, nutrition or immune deficiencies, malignant neoplasms, Diabetes, or other cardiovascular diseases
- Educate patients/families of cardiovascular conditions and/or procedures while being mindful of patient’s emotional, cultural and physical requirements
5. Promote optimal health among patients/families through preventative measures involving counseling, screenings, as well as disease prevention and management strategies
6. Assess wounds to provide proper diagnosis, including measurement of wound size, description of characteristics of wound bed as well as risk factors that may impact wound healing
7. Develop wound treatment orders based on best-practices, evidence-based guidelines and effective collaboration with interdisciplinary team
8. Perform wound treatment and dressings that include specialized interventions such as mechanical, or conservative sharp debridement as well as monitor and evaluate effectiveness of interventions to adjust treatment plan as needed
9. Demonstrate a commitment to personal and professional development by staying abreast of emerging wound care advancements in techniques, pharmacologic treatment, and research to provide optimal wound management
10. Function as a liaison between physicians and nursing staff ensuring effective communication to enhance quality and continuity of care
11. Create an environment that fosters collaboration through leading in-services or participation in quality performance initiatives
12. Educate both patient and families on disease prognosis and treatment plan, as well as foster therapeutic relationships with patient and families to establish trust in medical care and encourage compliant
JOB REQUIREMENTS
Graduate of a master’s degree program or higher from an accredited school for nurse practitioners in the specialty of Family medicine (FNP), or Adult medicine (Adult-Gerontology)
Licensures/Certifications: Current state Advanced Practice Nurse license. Certified by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP). CPR certified. In progress or has IL Controlled Substance License (CSL) and/or DEA license.
EXPERIENCE:
Previous experience in internal medicine, or family practice; an accredited wound care certification such as WOCN is ideal but not required
