As a Clinical Care Manager, you will:
- Coordinates and integrates patient care provided by agency staff, including contractors.
- Completes workflow tasks, administrative tasks, and other assignments to the CCM-RN responsible position in the agency’s electronic medical record. Uses add-on solutions per agency policy.
- Receives and enters orders from all relevant physicians, nurse practitioners, and other healthcare providers involved in the plan of care and ensures the orders are approved by the responsible provider.
- Assist in the entry of patient referrals, as needed.
- Follows up on pending referrals to ensure timely admission in collaboration with the intake coordinator.
- Reviews patient occurrence reports and takes appropriate action.
- Reviews patient infection control reports to ensure appropriate action has been taken and items are reported to the QAPI committee.
- Reviews on-call coordination notes and takes appropriate action.
- Assures that patient needs are continually assessed. Assures reassessment of the patient’s needs is performed by the appropriate healthcare professional when there is a significant health status change in the patient’s condition, at the physician’s request, and/or after hospital discharge.
- Assures the development, implementation, and updating of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s)
- Ensures provision of services as ordered by the physician as indicated in the plan of care, including patient and caregiver education and patient, caregiver, and family counseling.
- Performs in-home visits as assigned.
- responsibility for continuity, quality, and safety of services delivered in compliance with State and federal regulations.
- Monitors systems and identifies problem areas to branch director both verbally and through written reports.
Skills for Success:
- Ensures effective communication with all physicians involved in a patient’s plan of care and other healthcare practitioners (as appropriate).
- Prepares clinical notes as appropriate.
- Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
Additional Duties
- Prepares for and participates in case conference in collaboration with other healthcare team members and participates in decision-making regarding patient recertification/discharge.
- Meets mandatory continuing education requirements of the agency and licensing board.
- Promotes and educates regarding the concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
- Participates in agency-sponsored in-service training.
- Performs other duties and activities as delegated by the branch director.
Experience to Deliver on Our Mission:
- Registered nurse with current licensure to practice professional nursing in the state.
- Graduate of accredited school of nursing. BSN degree is preferred but not required.
- Two years management or supervisory experience in healthcare or home care setting, preferred.
- Complies with accepted professional standards and practice.
- Possesses and maintains current CPR certification.
- Demonstrates excellent observation, verbal, and written communication skills.
