As the Patient Services Coordinator, you will:
- Schedule the patient services and coordinate with the patient, family and care team members.
- Ensure that all agency field personnel are properly scheduled to ensure expected levels of productivity are achieved and care is provided in the most efficient manner possible.
- Ensure field caregivers have the information they need to appropriately care for the patient.
- Provided care to patients as needed

Experience to Deliver on our Mission:
- High school diploma, valid state driver’s license and auto liability insurance
- Current Vocational/Practical Nurse License preferred
- One year experience in home health or hospice preferred
