The QOS is responsible for overseeing quality and outcome initiatives within their assigned region and for coordinating the organization-wide outcomes measurement and reporting program in accordance with current regulations. Works with clinicians and branch leaders to improve clinical documentation, accuracy of OASIS
comprehensive assessments and tying assessments back to quality outcomes that contribute to Value-based care and publicly reported outcomes. The QOS you will coordinate, analyze, review, and educate staff and agency leadership regarding accuracy and compliance of all clinical data related to improving outcomes and process measures. Collect and analyze real-time quality data used in benchmarking and/or identification of best practice and performance improvement efforts. Quarterly summary reviews will be provided with all locations with performance improvement plans focused on quality measures each quarter. The QOS will perform onsite
visits to branches to provide group and individual training.
Essential Job Functions/Responsibilities
- Responsible for education related to home health and hospice practices in accordance with regulations and internal policies. Serves as a resource to regional and local leaders.
- Analyzes, reviews, and educates branch staff to assure clinical documentation is complete, consistent with care delivered, meets regulatory standards, and is focused on improving outcomes.
- Offers recommendations on electronic medical records, e.g., workflow, coordination notes, to improve documentation on care provided.
- Uses effective interpersonal and communication skills to achieve desired outcomes.
- Assists in the training and use of add-on applications with a goal of streamlining operational processes and improving care delivery.
- Maintains current evidence-based clinical guidelines and best practices, organization’s protocols, and policies. Collects and analyzes data and works with local leaders to identify areas for improvement in relation to:
- CAHPs;
- OASIS specific QRP measures (HH)
- HOPE and HCI measures (HS)
- Value Based
Purchasing metrics (HH) - Outcome and STAR ratings ((HH)
- OASIS and documentation deficiencies (HH)
- Process measure improvement.
- Based on performance and real time data, all support provided to agencies will be determined using a tiered quality system in which the QOS will use to ensure a focused approach to support and education is provided to assigned agencies by the QOS. At a minimum, the QOS will have interaction and engagement with all assigned branches monthly or more based on need (a) Tier 1 agency – Highest rating. Minimum Q30 day engagement; (b) Tier 2 agency – Minimum Biweekly engagement; and (3) Tier 3 agency – Minimum weekly engagement
- Assumes responsibility for continuity, quality, and safety of services in compliance with regulations.
- QAPI support to agencies.
- Fosters collaboration and communication among clinical and non-clinical teams to meet company goals.
- Fosters a positive and collaborative work environment including operations, clinical, and quality team
members. - Maintains professional and technical knowledge through ongoing education and workshops.
Position Qualifications
- Licensed Registered Nurse or Physical Therapist, preferred. Must be licensed in all states of assigned territory.
- Current driver’s license in state of residence, reliable transportation, and current auto liability insurance.
- Minimum of 3 years’ experience in QAPI, clinical leadership or education in home health or hospice, preferred.
- Knowledge of OASIS and/or HIS/HOPE data collection required
- Homecare Homebase experience preferred
- Ability to travel up to 50% of time within assigned region.
- Understanding of publicly reported quality and patient satisfaction metrics.
- Demonstrates a current working knowledge of Medicare guidelines and the “Conditions of Participation” for Home Health or Hospice.
- Proficiency in Microsoft Teams, Excel, Word, and Outlook; adept at learning new software programs
- Excellent communication and people skills.
