About the Role

The ideal candidate will approach each day with a passion for caring for patients by working one-on-one with them, helping them regain independence, and achieving health goals.

Key Responsibilities:

• Collaborate with the RN to fulfill patient-specific care plans
• Work with the care team to ensure all patient needs are fully addressed
• Deliver high-quality skilled care to patients
• Document care delivery daily in our electronic medical record (EMR) system
• Exhibit compassion and focus on results in care delivery

Candidates must possess a current vocational/practical nurse license, valid state driver's license, and auto liability insurance. One year of experience as a licensed vocational/practical nurse in a clinical care setting is required, with home health experience preferred.
