Job Responsibilities

- Assess patients' physical, emotional, and social needs and develop interventions to address these needs
- Communicate effectively with patients, families, and caregivers to ensure seamless care transitions
- Participate in ongoing education and training to stay current on best practices in home health care
Requirements
- Current RN license valid in state of practice
- Valid state driver's license and reliable transportation
- One year of experience as a Registered Nurse in an acute or clinical care setting preferred. Home health experience preferred
