Job Summary
We're looking for an Occupational Therapist who shares our values of being Trustworthy, Capable, Compassionate, Proactive, and most importantly, Called to serve patients in their home.
Key Responsibilities
- Evaluate and assess patient to determine their appropriateness for care
- Develop patient-specific care plans with the appropriate level and frequency of care
- Communicate with the care team, physician, payors, patients, and family to address care needs and fulfill the patient's care plan goals
- Deliver high-quality skilled care and supervise the care of other care team members
- Document care delivery daily in our EMR system
Requirements
- Current Occupational Therapist License, valid state driver's license, and auto liability insurance
- One year of experience as an Occupational Therapist in an acute care, rehabilitation, or home health setting preferred
