Job Description:
VitalCaring is seeking a Patient Services Coordinator, LN to serve as a bridge linking patients to home health care services. This role requires coordination of details between the patient, provider, and our care team members to ensure timely care and smooth transitions.
Key Responsibilities:
- Schedule patient services and coordinate with patients, family, and care team members
- Ensure agency field personnel are properly scheduled for expected levels of productivity and efficient care delivery
- Provide necessary information to field caregivers for appropriate patient care
- Provide care to patients in their homes as needed
Requirements:
- Current Vocational/Practical Nurse License and valid state driver's license with auto liability insurance
- One year experience as a Licensed Vocational/Practical Nurse in a clinical care setting, with home health experience preferred
