Job Title: Physical Therapist - Home Health
We are seeking a passionate and compassionate Physical Therapist to join our team at VitalCaring. As a home health physical therapist, you will have the opportunity to work one-on-one with patients in their homes, helping them regain their independence and achieve their health goals.
Responsibilities:
- Evaluate and assess patients to determine their appropriateness for care
- Develop patient-specific care plans with the appropriate level and frequency of care
- Communicate with the care team, physician, payors, patients, and families to address care needs and fulfill the patient's care plan
- Deliver high-quality skilled care and supervise the care of other care team members
- Document care delivery daily in our EMR system
- Contribute to a culture of caring through individual accountability and teamwork
Requirements:
- Current Physical Therapist License, valid state driver's license, and auto liability insurance
- One year of experience as a physical therapist in an acute care, rehabilitation, or home health setting preferred
