Coverage Area :

Far Northern Lke County Illinois, including Round Lake, Lake Villa and Antioch areas.

Great pay at $73 / hr (2,920 / wk) with weekly direct deposit!

HH Case Manager RN Contract

13 weeks with extension options

HH Case Manager RN Shift

Day shifts

8 hour shifts (8 : 00 am-4 : 30pm)

some weekend rotation possible

No on-call

Required Certifications : BLS, IL RN license

Experience : 1 year Home Health RN experience

Charting System : EPIC (experience required)

POSITION DESCRIPTION :

Care consisting of education on medications, disease management and self care along with performing skills such as wound care, infusion, line maintenance, ostomy care, etc and teaching patients how to perform care for these as well. This position is 100% in the field, with nurse driving from one patient to the next. There is no office to report to, all meetings held virtually. Initial trainig required on site in Oakbrook for 2 days only. Expectation is for 6 routine visits per day.
