Clinical Assessment Nurse Responsibilities:
- Conduct initial, quarterly, and change of condition assessments in order to continually meet resident needs and evaluate the capability of the community to meet those needs
- Encourage resident participation in community activities
- Based on resident assessments, coordinate services of the third party providers (hospice, home health, etc.)
- Identify and document resident needs and actions taken to meet resident needs including, but not limited to: care plans, aide care plans, medication assistance, fall management, and change in condition
- Participate in care plan meetings with Director of Wellness, families, or responsible parties to foster good communication, exemplary resident care, and great customer service
- Participate in the clinical on-call rotation along with the Director of Wellness and other clinical staff such as the Wellness Supervisor or Wellness Coordinator
- Assist with quality assurance audits, as requested
- Perform other duties as assigned by Wellness Director

- LPN with current state license
- At least 1 year of full-time experience in a senior living environment
- Proficient knowledge of geriatric and dementia care nursing
- Must have a valid driver's license or availability of transportation to perform job related duties including but not limited to performing off-site assessments
- Demonstrated ability to communicate effectively in English, both verbally and in writing
- Able to lift up to 50 pounds
