Our programs provide intensive support for individuals experiencing moderate to severe mental health concerns through structured outpatient treatment. We are seeking a Case Manager to join our team, working in collaboration with our multidisciplinary clinical team to provide case management, resource coordination, and psychoeducational support for our clients.

We are dedicated to providing innovative, evidence-based mental health care through Intensive Outpatient (IOP) and Partial Hospitalization (PHP) programs, helping clients build sustainable wellness and recovery plans.
ESSENTIAL FUNCTIONS:
- Provide case management services to a caseload of 10-12 clients in IOP/PHP.
- Conduct psychoeducational groups focused on life skills, coping strategies, and recovery support.
- Collaborate with clients, families, and other support systems to develop individualized care plans.
- Identify community resources, including educational, vocational, and housing support, to enhance client stability and recovery.
- Assist clients in navigating healthcare systems, including referrals for medical, psychiatric, and therapeutic services.
- Maintain accurate and timely documentation of case management interventions, progress notes, and treatment plans within the Electronic Health Record (EHR) system.
- Participate in multidisciplinary team meetings to ensure coordinated and comprehensive care.
- Support clients in developing academic, vocational, and independent living goals that align with their mental health recovery.
- Ensure compliance with state and federal regulations, HIPAA standards, and ethical guidelines for case management.
- Work collaboratively with therapists, psychiatrists, and discharge planners to facilitate smooth transitions in and out of care.
- Provide crisis support and intervention as needed, escalating cases appropriately within the clinical team.
- Conduct outreach to identify speakers, community groups, and resources that can benefit clients in the program.
QUALIFICATIONS:
- Education: Bachelor’s degree in Psychology, Social Work, Counseling, or a related field (preferred).
- Experience: Minimum 2 years of experience in mental health, advocacy, or case management.
- Certifications:
- RADT-I certification preferred (or willingness to obtain).
- CPR/First Aid certification required.
- Skills & Competencies:
- Strong knowledge of mental health disorders and evidence-based treatment approaches.
- Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint) and Electronic Health Records (EHR) systems.
- Strong documentation, organization, and time-management skills.
- Ability to effectively collaborate within a multidisciplinary team.
- Strong verbal and written communication skills.
- Other Requirements:
- Valid Driver’s License and reliable transportation.
- Negative TB test results and medical clearance within the past year.
- Ability to pass pre-employment and random drug screenings.
WORK ENVIRONMENT & PHYSICAL REQUIREMENTS:
- Ability to sit, stand, walk, and perform repetitive tasks for extended periods.
- Ability to lift and carry up to 20lbs occasionally.
- Moderate frequency of computer use and documentation.
- Office environment with regular exposure to computer screens and office equipment.
- Occasional interaction with clients experiencing emotional distress.
COMPENSATION & BENEFITS:
- Salary Range: $19 - $25 per hour (based on experience and qualifications).
