What are the responsibilities and job description for the LPN/LVN Care Coordinator position at VITAS Healthcare?
In this role, you will be responsible for providing direct skilled bedside nursing care to terminally ill patients admitted to the VITAS program. You will evaluate patient/family needs and assist in the preparation and execution of the interdisciplinary plan of care for each patient on the team.
Restrictive Guidelines in Care Delivery:
- Does NOT make initial patient nursing care assessments.
- Supervised by a Registered Nurse
- Reports to the applicable Team Manager
