**Job Description:**
We are seeking a skilled and compassionate Registered Nurse to join our team as a Terminal Illness Care Coordinator at Vitas Healthcare. In this role, you will work closely with patients and families to develop and implement individualized plans of care that address their physical, medical, psychosocial, emotional, and spiritual needs.
Responsibilities:
- Assesses patients' and families' needs and develops individualized plans of care
- Coordinates care with other healthcare professionals to meet the physical, medical, psychosocial, emotional, and spiritual needs of patients and families facing terminal illness and bereavement
- Supervises Hospice Aides, Licensed Practical/Vocational Nurses, and Volunteers
- Participates in Quality Assessment and Performance Improvement (QAPI) Program
Requirements:
- Current licensure as a Registered Nurse in the state where the VITAS program is located
- A minimum of two years of nursing experience in hospice, home health, or community health in the last five years
