Vitas Healthcare is seeking an experienced RN Care Navigator to support our patients and families through the complex healthcare system. Main Responsibilities:
* Evaluate patients for hospice eligibility and obtain necessary documentation
* Communicate with patients and families to provide education on hospice services and benefits
* Collaborate with healthcare professionals to ensure seamless transitions of care
* Analyze data to identify trends and opportunities for improvement
* Develop and maintain strong relationships with referral sources and healthcare partners
What We're Looking For:
* 2 years of experience as a registered nurse in a hospital or healthcare setting
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Proficiency in electronic medical records and other software applications
* Reliable transportation and a valid driver's license
Education:
* Bachelor's degree in nursing required
