Responsibilities:

Qualifications:
- Valid Physical Therapy license
- Minimum 6 months Physical Therapist experience
- 1 years of Home Health experience is a plus.
- CPR or BLS Certification
- Recent Physical and TB Test (within one year)
- Willingness to travel within a 20-30 mile radius
- Valid Driver's License or State ID
- Full COVID vaccination with booster shot
