Job Summary
We are seeking a compassionate and skilled Visiting Nurse to provide high-quality nursing care to patients in their homes. The ideal candidate will have experience in a variety of medical settings, including Level I and Level II Trauma Centers, and possess a strong understanding of medical-surgical nursing. This role requires the ability to assess patient needs, implement care plans, and collaborate with healthcare teams to ensure optimal patient outcomes.
Responsibilities
- Conduct comprehensive assessments of patients' health status in their home environment.
- Develop, implement, and evaluate individualized care plans based on patients' needs and goals.
- Administer medications and treatments as prescribed by physicians.
- Provide education to patients and their families regarding health management, including medical terminology and physiology knowledge.
- Assist with specialized care such as ostomy care and family planning services.
- Collaborate with interdisciplinary teams to coordinate care and improve patient outcomes.
- Maintain accurate documentation of patient progress and interventions in compliance with healthcare regulations.
- Ensure adherence to safety protocols while providing care in various home settings.
Qualifications
- Registered Nurse (RN) license valid in the state of practice.
- Experience in medical-surgical nursing or related fields preferred.
- Knowledge of systems used in healthcare delivery, including electronic health records (EHR).
- Familiarity with procedures related to dialysis is a plus.
- Strong understanding of medical terminology and physiology knowledge is essential.
- Excellent communication skills, both verbal and written, for effective patient education and collaboration with healthcare teams.
- Ability to work independently while managing multiple patient cases effectively.
