The Team Lead, Care Coordination is responsible for providing support, oversight, and clear direction to Health Services staff to achieve departmental goals and objectives. This individual will be responsible for working with management to coordinate ride-a-longs, training, and staff meetings, maintaining an active caseload, and providing non-clinical services for Medicaid recipients to improve the management of a chronic disease or other populations as identified by the Alabama Medicaid Agency (AMA). These services are provided in the home, via phone, and/or in person. This role is responsible for education, counseling, follow-up, and resolving transportation issues, ensuring services are delivered and continuity of care is maintained. This position will travel to locations within the relevant service area through a reliable means of transportation insured by Company policy. This position will have occasional work-from-home opportunities.
REQUIRED:
- BSW, BSN, or equivalent experience
- One year of experience in case management, human services, public health, or experience with the underinsured population
- Current RN or SW license in good standing with the appropriate State of Alabama Board
- Valid driver's license in good standing
- May require significant face-to-face member contact with duties regularly performed away from the principal place of business
- Willing to submit to vaccine testing and screening
- Excellent interview and telephone skills, as well as the ability to deal with recipients in a caring and helpful (assertive and tactful) manner
- Ability to be flexible, adaptable, and accommodate shifting priorities while working effectively in a variety of settings
- Ability to perform day-to-day responsibilities with minimal supervision, exercise independent judgment, and maintain confidentiality
- Excellent communication, relationship, organizational, and time management skills
- Working knowledge of health-related service delivery systems
- Ability to analyze varied environmental factors concerning members’ wellbeing
- Basic computer skills
PREFERRED:
- MSW
- Four years of experience in case management, human services, public health, or experience with the underinsured population
- Certified Case Manager (CCM)
