POSITION TITLE : CARE MANAGER II - HHA
DEPARTMENT : Clinical Services
REPORTS TO : Wellness Director
POSITION SUMMARY :
- The essential functions of the job for the Care Manager II-HHA, requires, and may be amended from time to time at the sole discretion of Viva Senior Living to include, but are not limited to the following :
- Comply with standards of business conduct in accordance with federal, state and local health and regulatory standards and guidelines, as applicable.
- Must possess, at a minimum, a high school diploma or a GED
- Must be able to work well with peers in a congenial work environment, and above all, maintain the utmost compassion and dignity to the residents and adhere to the Resident Bill of Rights.
- In exigent circumstances, the employee may be subject to call back during emergency conditions (e.g., severe weather,
evacuation, temporary staffing shortages, etc.).
Must adhere to Viva Senior Living's attendance policies and follow the terms and conditions of employment as set forth in the Employee Handbook.Must be able to read, write, speak and understand English.Must maintain the highest standards in caring and servicing the needs of the residents and residents' family members and loves ones.Must adhere to all facility policies and procedures.RESPONSIBILITIES / ACCOUNTABILITIES
Supervise and care for residents on daily basis according to your work list and assignmentProvide assistance with activities of daily living according to resident's individualized Care Plan, regarding bathing, shower, shampoo if necessary, make bed, linens, escorts, assistance with dressing, oral hygiene, shaving, and hair and fingernail care if neededObserve and record assistance with prescribed medications, as per assignmentReport refusals and / or reactions to care or medications to supervisor on dutyDirect residents and family members to appropriate staff for other needs or informationAssist with ordering room service for residents, when neededReport and record observations of change in resident condition to director / supervisor and transfer relevant information to the next shift through the 24-hour shift reportBe patient and understanding when communicating with staff, residents, and visitors. Speak slowly and clearlyProvide assistance in dining room, as neededAnswer emergency calls PROMPTLY and courteously, and complete required paperwork afterwardRetrieve soiled linen and personal clothing from resident living areas. Sort and pre-soak linens, as required, before processing through washing machines, per resident's Plan of CareResponsible for general upkeep and cleanliness of a resident's apartment / room. Empty waste baskets, wipe up spills, wash dishes, put away clothes, check refrigerator for spoiled foodReport any unsecured throw rugs, extension cords, and any other fire / fall hazards to maintain the No Fall ZoneObserve condition of room / apartment and report anything in need of repair to maintenance / supervisorDocument clearly and objectively on residentsParticipate in in-service educational programs, as required by the community and state.Record all care given and refusalsMaintain required safety / building checksObtain resident weights, as neededMaintain universal precautions; bloodborne pathogens and biohazard precautionsPromote positive public relations for the CommunityProvide assistance with toileting, personal care, changing incontinence productsAssist with use of durable medical equipment (DME) to include walkers, wheelchairs, and canesUse good body mechanics to provide occasional assistance with transfersAssist residents to perform passive range of motion (ROM) exercises, as per Plan of CareAssist resident with braces, splints, slings, hearing aides, artificial breasts, dentures, and TEDS stockings, according to Plan of CarePerform other duties, as requiredRequirements

Must provide annual verification of a negative TB skin test.

Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, agitated residents
Must be able to push, pull, move, and / or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and / or carry such weight a minimum distance of 50 feet.
May be necessary to assist in the evacuation of residents during emergency situations.
