Scope
As a Community Engagement Specialist, you will be a trusted resource, advocate, and educator, helping individuals navigate the healthcare system, access essential services, and build healthier communities. Your work will focus on community outreach, health education, and administrative support, ensuring that diverse populations receive the care and resources they need.
As a Community Engagement Specialist, You Will
- Engage & Educate the Community – Help develop and conduct outreach efforts to promote health and preventive care. Offer culturally relevant health education tailored to Hispanic communities.
- Support Individuals & Families – Help navigate healthcare systems, schedule appointments, arrange transportation, and connect individuals with community resources.
- Facilitate Data Collection & Reporting – Track community health trends, assist in data collection, and support population health initiatives through organized reporting.
- Provide Administrative Support – Assist with scheduling, communications, event planning, and general office tasks to keep the Population Health Department running smoothly.
- Other Duties as Assigned – Build and maintain relationships with local organizations, healthcare providers, and social service agencies to expand available resources. The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
- High school diploma or equivalent; additional education in public health, social work, or a related field preferred.
- Certification as a Community Health Worker (CHW) is required or must be obtained within six months of hire.
- Experience with developing and implementing community health programs.
- Experience building and sustaining collaborative community partnerships.
- Fluent in English and Spanish (spoken and written), with demonstrated cultural competency in working with Hispanic communities.
- Organizational and time management abilities.
- Ability to pass state mandated background check.
- Valid driver’s license and ability to travel for community outreach activities.
- Physical capability to perform all essential job functions.
