Scope
Assume role of the charge nurse to supervise resident care as assigned.
B. Responsible for direct care, monitoring and evaluation of services provided to residents.
D. Facilitate a verbal and written communication system with Health Services Director, Complex
Supervisor, caregivers, physicians and family members to ensure effective, continuous resident
care and high levels of customer satisfaction.
F. Collaborate with the Corporate Quality Improvement staff and participates in quality
improvement initiatives.
G. Support the marketing efforts of the community and its programs.
H. Perform other duties as assigned by the Director of Health Services or designee.

Scope
- Responsible for the administration of general care of the residents and patients.
- Provide regular work direction and monitors the work of Nursing Assistants and Care Attendants on assigned shift.
- Works closely with the patient/resident and with various members of other departments such as activities, meal services, social services, chaplain, housekeeping, and other members of the nursing department.
- The LPN may have contact with the family of the resident/patient.
- Must have flexibility in schedule to fulfill an on-call rotation if required and be available for on-call medical questions and/or emergencies via phone or in person as needed.
- Current/Active License as an LPN in the State of Minnesota required. Current BLS Certification or ability to obtain within 30 days.
- Preferred Experience: One year assisted living or long-term care experience preferred. Previous experience working with a senior population preferred. Previous experience working with an Electronic Medical Record. Previous experience caring for residents with dementia.

- Monitor Resident Assistant performance, ensures completion of resident cares, and

- Notifies Director of Health Services or designee of Resident Assistant performance issues in

Supervisor, caregivers, physicians and family members to ensure effective, continuous resident
care and high levels of customer satisfaction.
- Changes in resident conditions are communicated to on-call RN, Complex
- Supervisor, Supervisor in a timely manner.
- Contacts the physician/NP per established procedures for changes in resident condition.
- Monitor to ensure resident rights are maintained and notify the immediate supervisor of
- Knowledgeable of the Vulnerable Adult Prohibition policy and reporting requirements when
