Key Responsibilities
- Support communities and drive individualized, resident-centered care delivery systems to define needs and facilitate solution strategies utilizing knowledge of the nursing process, theory, principles, technology, and evidence-based practice while sustaining compliance with company policies, as well as state and federal regulations.
- Travel to metro and surrounding areas, and parts of Wisconsin, to attend monthly site visits, training, meetings, and conferences, as directed. Travel required 50% -75% of the time, with mileage reimbursement, and occasional availability to work from home. Flexibility to grow and expand region.
- Promote change and growth and inspire the development of a highly effective team of nurse leaders. Foster a workplace that results in a high performing team.
- Ensure continuity in approach and application of company policies and procedures while ensuring compliance with federal, state, and local regulations.
- Assist community Executive Director (ED) or Administrator with the onboarding and training of newly hired DOCC/DON/RN Managers.
- Participate in the opening of new/managed communities, as assigned.
- Monitor internal practices to ensure that incident and accident reports, medication errors, formal concerns, and other potential vulnerable adult concerns are identified, investigated properly, and appropriate corrective action is taken, when applicable.
- Educate teams on infection prevention and practices and provide guidance on implementation.
- Review assessment reports and perform package audits to ensure revenue is captured for services provided and review findings with the community leaders.
- Review and monitor adherence of reporting guidelines to the Minnesota Adult Abuse Reporting Center (MAARC), Office of Health Facility Complaints (OHFC), and Adult Protective Services (APS) in Wisconsin, participating in investigations as needed.
- Actively participate in survey readiness activities and provide active onsite support during a state survey and OHFC visits. Provide recommendations for Plans of Correction (POC) as needed.
- Participate in vaccination and testing clinics as directed by supervisor.
- Work collaboratively with community clinical leadership to support Quality Assurance and Performance Improvement initiatives in efforts to attain and maintain high clinical standards. Take a leadership role in company-wide quality reviews.
- Update and provide ongoing communication of issues/concerns regarding quality of care and compliance to the Senior Director of Clinical Services.
- Participate in and/or lead identified projects as assigned by supervisor. Participate on internal and external industry committees either in a lead or participant role.
- Build and maintain an atmosphere of Servant Leadership by developing respect and trust with team members, contract professionals, and outside providers.
- Collaborate and communicate regularly with other regional and department leaders.

- Current unrestricted license as Registered Nurse in the State of Minnesota is required. Secondary RN licensure in Wisconsin, or ability to obtain.
- Current BLS Certification.
- A minimum of three years' work experience in a clinical leadership position in Assisted Living (AL), and at least one year of experience in a Long-Term Care (LTC), Skilled Nursing Facility (SNF) or Transitional Care Unit (TCU), highly preferred.
- Strong knowledge of MN statutes and CMS guidelines for Assisted Living and Long-Term Care environments.
- Able to respond to emergency calls promptly, with sound judgement and follow-through.
- Organized, flexible, and detail oriented.
- Experience with a variety of Electronic Health Record (EHR) software systems and a strong working knowledge of Microsoft Office Suite applications is a must.
- Excellent listening, verbal, and written communication skills. Required to have the ability to read, write and speak English well enough to communicate with staff, residents and visitors.
- Outstanding customer service skills, with the ability to lead a team towards positive outcomes for residents and team members.
