Job Summary: This position is the designated individual responsible for the community’s infection control program and ongoing compliance. It will be combined with Admissions and MDS.
Responsibilities Include
- This program includes, at a minimum, a system for preventing, identifying, reporting, investigating, and controlling infections and communicable diseases for all residents, staff, and visitors
- Participate in the ongoing, periodic review of the infection control program and related policies and procedures
- Provide surveillance of infection control outbreaks in the community and takes appropriate actions to control the potential spread of infection
- Maintain a system for recording incidents identified under the IPCP and corrective actions taken
- Communicate surveillance reports to the Medical Director, keeps him/her informed of suspected outbreaks
- Utilize the results of the “facility assessment” to establish and update the IPCP
- Participate on quality assessment and assurance committee on a regular basis
- Provide infection related reports at the communities QAA committee
- Identify and report on compliance related concerns, work with interdisciplinary team to identify root causes, and move to correction
- Work in collaboration with the community’s administrator to update the “facility assessment” when there are changes in community practice or standards
- Handling admission, transfers and discharges of residents
- Complete related sections on the MDS/RAI
- Coordinate Medicare charting schedule
- Assessing and monitoring patient health to meet CMS compliance requirement
- Developing, implementing, and evaluating care plans using the nursing process to optimize patient wellbeing
- Communicating and collaborating with staff and patients to ensure appropriate care is given, addressing any incidents or complaints promptly
- Ability to recognize significant changes in the condition of residents and to take necessary action
- Helping patients enroll in federally funded Medicare and Medicaid health benefits
- Other duties and responsibilities assigned by DON or ADON within the scope of Minnesota Nurse Practice Act
- Graduated from an accredited Registered Nursing program.
- Specialized training in infection prevention and control and/or be qualified by education, training, experience, or certification.
- Must be licensed as a Registered Nurse in the state of Minnesota, with no restrictions.
- Ability to communicate effectively in both verbal and written formats.
- Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precautions, AWAIR, OSHA, Right to Know, SDS, ADA, and CMS guidelines.
- Minimum of 1 year of experience in a SNF or LTC facility with experience in Infection Control, MDS and admissions, preferred.
- BSN preferred, but not required.
