DIRECTOR OF SOCIAL SERVICES OVERVIEW :
The primary purpose of your job position is to plan, organize, develop, and direct the overall operation of our facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met / maintained on an individual basis.
SPECIFIC REQUIREMENTS :
Must be able to read, write, speak, and understand the English language.
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
Must be a supportive team member, contribute to and be an example of team work and team concept.
Must be knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies / personnel and the general public.
Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning.
Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services.
Must be a member in good standing in the National Association of Social Workers and Academy of Certified Social Workers, Inc.
Must not pose a direct threat to the health or safety of other individuals in the workplace.
WORKING CONDITIONS :
Works in office areas as well as throughout the facility.
Moves intermittently during working hours.
Is subject to frequent interruptions.
Is involved with residents, personnel, visitors, government agencies / personnel, etc., under all conditions and circumstances.
Communicates with the medical staff, nursing service, and other department directors.
Works beyond normal working hours, on weekends, and in other positions temporarily, when necessary.
Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
Attends and participates in continuing educational programs.
Maintains a liaison with other department supervisors to adequately plan for social services / activities.
PERFORMANCE EVALUATION CRITERIA :
Plan, develop systems to organize, implement, evaluate, and direct the social service programs of this facility.
Participates in discharge planning, development and implementation of social care plans and resident assessments.
Reports and investigates all allegations of Abuse, neglect and exploitation. Maintains Abuse binder with all resident abuse allegations logged with completed investigations. Reviews with QA&A committee every month any identified trends.
Assures that social service progress notes are informative and descriptive of the services provided and of the resident’s response to the service.
Serves on, participates in, and attends various committees of the facility (i.e., Infection Control, Standard of Care, Quality Assessment and Assurance, etc.) as required, and as appointed by the Administrator. Facilitates monthly psychoactive / behavior meeting.
Maintains a productive working relationship with other department supervisors and coordinate social services to assure that daily social services can be performed without interruption.
Implements, and maintains an effective orientation program that orients the new employee to the department, its policies and procedures, and to his / her job position and duties.
Develops a written plan of care for each resident that identifies social problems / needs of the resident and the goals to be accomplished for each problem / need identified.
Develops and maintains a good rapport with all services involved with the care plan to ensure that a team effort is achieved in developing a comprehensive plan of care.
Maintains a written record of the resident’s complaints and / or grievances that indicates the action taken to resolve the complaint and the current status of the complaint. Tracks and Trends all grievances and reports identified trends to the QA&A committee monthly.
EDUCATION & EXPERIENCE :
Must possess, as a minimum, a Bachelors Degree from an approved school of Social Work.
Prefer, as a minimum, two (2) years experience in a supervisory capacity in a hospital, nursing care facility, or other related medical facility.
PHYSICAL AND SENSORY REQUIREMENTS (WITH OR WITHOUT THE AID OF MECHANICAL DEVICES) :
Must be able to move intermittently throughout the workday.
Must be able to speak and write the English language in an understandable manner.
Must be able to cope with the mental and emotional stress of the position.
Must possess sight / hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
Must be able to push, pull, move, and / or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and / or carry such weight a minimum distance of 50 feet.
May be necessary to assist in the evacuation of residents during emergency situations.
