VNA Alliance is a community-based, non-profit, Medicare certified home health agency based in Meadville, PA. VNA Alliance currently serves Crawford, Mercer, and portions of Venango county and offers skilled nursing, palliative care, and therapy services. In addition to the mentioned, VNA Alliance offers additional medical services and implemented a Safe at Home program that is a clinical pathway to optimize outcomes in the acute care and home health care setting. The Team Assistant lll assists and performs various duties to support daily operations that may include coordinating, monitoring and evaluating options and services for patients, clinical staff and referral sources
RESPONSIBLE TO
Clinical Manager
Executive Director
QUALIFICATIONS
A. Reviews incoming documentation for compliance and completeness
B. Enters patient demographic and clinical data into current electronic medical record and software programs, as required
C. Assists in preparing management reports and gathering relevant statistical data of patient’s information as requested
D. Handles phone calls or general inquiries and messages for staff, clients and referral sources in a positive, courteous, pleasant and professional manner
E. Alphabetizes paperwork and files/uploads documents into the electronic medical record system appropriately
F. Assists and coordinates with Education on new hires, equipment and onboarding calendars and other clerical duties.
Assists in clerical duties for the Quality Assurance Nurse
G. Answers basic questions to telehealth operation and troubleshooting
H. Generate and distributes patient information to physicians, patients, and other health-care providers to meet the Conditions of Participation
I. Provides backup to the Team Assistant 1 and Team Assistant 11
J. Other duties and patient-related tasks as assigned
COMMUNICATION
A. Communicates all changes in the daily schedule to the appropriate supervisor and/or staff member
B. Notifies all disciplines involved in patient’s care of transfers
C. Identifies problems/concerns with patients and staff and reports them appropriately
D. Handles general inquiries whether received by telephone, mail, or in person to and from staff, patients, and referral sources
E. Collaborates with health-related organizations to improve coordination of care and services to the community
F. Maintains confidentiality of patient and staff information following HIPAA
DOCUMENTATION
A. Exhibits a thorough knowledge of the agency’s electronic medical record and other software systems.
B. Able to demonstrate expertise in all aspects of required documentation.
C. Serves as a role model for all staff

QUALIFICATIONS
- High school education or equivalent required
- At least two [2] year experience in a health care related environment preferred
- Computer skills: Microsoft Word and Excel
- Excellent organizational skills with attention to detail
- Outstanding communication and interpersonal skills, both written and verbal
- Ability to prioritize and organize daily work load
- Must have a current and valid driver’s license. Reliable, independent means of transportation with vehicle insurance limits as established by the state
- Knowledge base of agency policies, federal and state regulations and third-party payers
- Ability to maintain professionalism under deadlines
